羊水分析诊断遗传病羊水分析诊断遗传病诊法。即在婚前或 孕前进行遗传咨询的基础上，结合羊膜腔穿刺术，通过对羊水中来自胎儿的脱落细胞和代谢 产物的分析，可对胎儿性别、胎儿成熟度、伴性遗传病、染色体核型与代谢生化等作出判断 。 ①遗传病宫内诊断的适应证：a.40岁以上的高年孕妇。因其有4.5%的可能性生育染色体异 常 儿，其中主要是21-三体综合征。b.有明显遗传病家族史的孕妇或近亲婚配者。c.羊水过 多 症。d.不明原因的多次流产或死胎。因在早期流产中染色体异常者20%～50%，死胎中占8% 。e .曾有开放性神经管缺陷或其他先天性代谢异常儿分娩史或血清甲胎蛋白异常增高的孕妇。 f .曾有染色体异常患儿分娩史。g.严重X-连锁隐性遗传病基因携带者。②把收集到的羊水 低 速离心，将它的上清液和沉淀细胞作以下分析。a.上清液：可明确胎儿红细胞增多症的严 重程度。可诊断神经管缺陷。胎儿羊水中的甲胎蛋白在妊娠13～16周时可高达25μg/ml，2 2周后开始下降。无脑儿及脊柱裂等神经管畸形儿，羊水中甲胎蛋白异常增高。可诊断先天 性 肾病，食道、肠闭锁，脐疝，骶尾部畸胎瘤。先天性卵巢发育不全。b.Rh血型不和等的甲 胎 蛋白含量也会增高。c.沉淀细胞：可预测胎儿性别。把收集的沉淀细胞直接固定后用硫瑾 紫 染色。如在细胞核内出现点状荧光，这就是Y染色原，如出现频率在20%～50%，可诊断为男 胎 ，0～3%为女胎。对一些X-连锁隐性遗传病如血友病、高尿酸血症、假性肥大型肌营养不良 症 等病，其特点是儿子患病而父母正常。女性胎儿的正常基因和致病基因携带的可能性各占1/ 2。在男性胎儿中，正常和患病的机率也各占1/2。沉淀细胞经小牛血清培养可作染色体核型 分析及酶学测定，从而诊断染色体病和先天性代谢缺陷病。d.培养细胞的染色体数目，形 态结构分析可诊断所有染色体异常疾病。
